Background:Inventralherniasurgery,meshimplantsareusedtoreducerecurrence.Infectionaftermeshimplantationcanbeaproblemandratesaround6–10%havebeenreported.Bacterialcolonizationofmeshimplantsinpatientswithoutclinicalsignsofinfectionhasnotbeenthoroughlyinvestigated.Moleculartechniqueshaveproveneffectiveindemonstratingbacterialdiversityinvariousenvironmentsandareabletoidentifybacteriaonagene-specificlevel.Objective:Thepurposeofthisstudywastodetectbacterialbiofilminmeshimplants,analyzeitsbacterialdiversity,andlookforpossibleresemblancewithbacterialbiofilmfromtheperiodontalpocket.Methods:Thirtypatientsreferredtoourhospitalforrecurrenceafterformerventralherniameshrepair,wereexaminedforperiodontitisinadvanceofnewsurgicalherniarepair.Oralexaminationincludedperiapicalradiographs,periodontalprobing,andsubgingivalplaquecollection.Apieceofmesh(1×1cm)fromtheabdominalwallwasharvestedduringthenewsurgicalherniarepairandanalyzedforbacteriabyPCRand16SrRNAgenesequencing.FrompatientswithpositivePCRmeshsamples,subgingivalplaquesampleswereanalyzedwiththesametechniques.Results:Agreatvarietyoftaxaweredetectedin20(66.7%)meshsamples,includingtypicaloralcommensalsandperiodontopathogens,enterics,andskinbacteria.Meshandperiodontalbacteriawerefurtheranalyzedforsimilarityin16SrRNAgenesequences.In17sequences,thelevelofresemblancebetweenmeshandsubgingivalbacterialcolonizationwas98–100%suggesting,butnotproving,atransferoforalbacteriatothemesh.Conclusion:Theresultsshowgreatbacterialdiversityonmeshimplantsfromtheanteriorabdominalwallincludingoralcommensalsandperiodontopathogens.Meshcanbereachedbybacteriainseveralwaysincludinghematogenousspreadfromanoralsite.However,othersitessuchasgutandskinmayalsoserveassourcesforthemeshbiofilm.Keywords:ventralhernia;mesh;implants;oralbacteria;16SrRNA;DNA(Published:29July2016)Citation:JournalofOralMicrobiology2016,8:31854-http://dx.doi.org/10.3402/jom.v8.31854
