U500 insulin for the management of severe insulin resistance in pregnancy

【 摘 要 】

Pregnancy exacerbates insulinresistance in diabetic women. Inpregnant women with severe insulinresistance (SIR), administration of largeamounts of insulin is often difficult andimpractical. U500 insulin is five timesmore concentrated than standard U-100insulin and can be a valuable tool in thetreatment of women with SIR. Theobjective of our study was to evaluateglycemic control and hypoglycemia inpregnant women treated with U500insulin. This retrospective cohort studyincluded pregnant women with SIR witha need for greater than 200 units ofU100 insulin per day at the University ofIowa Hospitals and Clinics between theyears of 2016 to 2020. Twenty-ninewomen with SIR were treated with U500and 28 were treated with U100.Following initiation of U500, improvements in mean fasting glucosewas noted (97.15 vs 123.72, p<.001).Compared to patients who receivedsolely U100, women who received U500had improvements in fasting (97.15 vs112.98, p=0.07) and post-breakfastblood glucose (127.70 vs 140.50,p=0.04). There was no significantdifference in maternal hypoglycemia inpre vs post U500 (12 vs 17, p=0.286)and in U100 vs U500 insulin (13 vs 17,p=0.399). Due to improved glycemiccontrol with U500, clinicians shouldconsider offering this to their pregnantpatients with SIR.
