Riesgo quirúrgico en pacientes mayores de 60 años

【 摘 要 】

Se realizó el estudio prospectivo de 364 pacientes mayores de 60 años operados en el servicio de Cirugía General del Hospital Clinicoquirúrgico "Hermanos Ameijeiras", desde mayo hasta diciembre de 1994. Al sexo femenino le correspondió el 55,2 % y al masculino el 44,8 %. El número de enfermos disminuyó con el aumento de la edad de 224 entre 60 y 69 años, 107 entre 70 y 79, a 33 en los mayores de 80. El análisis univariado demostró relación de las complicaciones y la mortalidad con la estadía preoperatoria (p < 0,0005), la presencia de neoplasias (p < 0,005) y la envergadura de la operación (p < 0,05). El índice de masa corporal mostró asociación con la mortalidad (p < 0,05). Con el análisis de la regresión logística se halló que el sexo (p = 0,0105) y la envergadura de la operación (p = 0,0003) influyeron en la probabilidad de complicaciones. El número de enfermedades asociadas (p = 0,0187) influyó en la probabilidad de mortalidad. La edad no demostró influencia sobre las complicaciones o la mortalidad (p 0,05). Se concluye en que la edad per se no debe constituir contraindicación para la cirugía
A prospective study of 364 patients over 60 years old that were operated on at the Service of General Surgery of the"Hermanos Ameijeiras" Clinical and Surgical Hospital from May to December, 1994, was conducted. 55.2 % were females and 44.8 % males. The number of patients decreased with age from 224 between 60 and 69 years old to 107 between 70 and 79 years old and to 33 over 80. The univariate analysis showed a relationship of the complications and mortality with preoperative stay (p < 0.0005), the presence of neoplasias (p < 0.0005) and the importance of the operation (p < 0.005). There was an association between the body mass index and mortality (p < 0.05). Through the logistical regression analysis it was found that sex (p < 0.0105) and the importance of the operation (p < 0.0003) influenced on the probabilites of complications. The amount of related diseases (p < 0.0187) also influenced on the probability of mortality. Age had no influence on the complications or on mortality (p < 0.05). It was concluded that age by itself should not be a contraindication for surgery
