【 摘 要 】

Background:Breastcoreneedlebiopsy(CNB)providesenoughtissueforhistopathologicdiagnosisandisconsideredareliablemethodforestablishingpreoperativetissuediagnosis.ThepurposeofthisstudyistoevaluateCNBasthefirstdiagnosticstepinsteadofexcisionalbiopsyinpalpablebreastmasses.MaterialsandMethods:Inthisprospectivestudy,patientswithpalpablebreastmasswhounderwentCNBwereenrolled.Basedonpathologyreport,patientswithmalignantlesionsrevealedbyCNBwereimmediatelycandidateforsurgeryandthosewhohadbenignlesionswerefollowedupto3years.Results:112femaleswithpalpablebreastmasswereenrolledinthestudy.In103(91.9%)ofcasesfirstattemptCNBprovidedadequatesampletissue.CNBdetectedmalignantlesioninseventyeight(69.6%)patients.All(100%)malignantCNBreportswereconfirmedatsurgeryspecimenpathology.In34(30.4%)patientsCNBrevealedbenignlesion.Attheendof3yearsfollowupperiod,25(73%)ofthesepatientsunderwentopenbiopsyleadingtothedetectionof1(3%)malignanttumor.Overall,accordingtothegoldstandarddefinedaspositivesurgicalbiopsyorpositivefollow-up,sensitivityofCNBwascalculatedas98.7%(95%CI,94.1-100%)anditsaccuracywas99.1%(95%CI,97.4-100%).ThespecificityoftheCNBprocedurewas100%.Conclusion:ThesefindingssuggestthatthemalignancydetectionpowerprovidedbyCNBmaybeweighedequaltothatofopenbiopsy.Therefore,weproposeCNBasthefirstchoiceindiagnosticevaluationofpalpablebreastmassespeciallyforthoseinaccessiblesitesandinexperiencedhands.
