The experience of chemotherapy with Topotecan in children with recurrent or refractory solid tumors

Introduction:Topotecan(Tp)isapprovedforadultswithdifferenttypesofmalignanttumors;howeverithasnotbeensufficientlystudiedinchildren.Purpose:TodeterminetheresponserateandtoxicityofthecombinationofchemotherapywithTpinpediatricptswithrecurrentorrefractorymalignantsolidtumors.Materialsandmethods:Between2005and201114pediatricptsreceivedTp-containingsecond-linechemotherapy.Eightptshadrelapseofneuroblastoma;3,rhabdomyosarcoma;1,malignantmesothelioma;1,synovialsarcoma,and1,arefractoryformofembryonalRMS(ERMS).Theirmedianagewas3.9years.ChemotherapywithTpwascarriedoutaccordingfollowingschemes:Tp1mg/m2+Carboplatin150mg/m2ondays1–4,Tp1mg/m2+Cyclophosphamide250mg/m2dayson1–7,andmonotherapywithTp1mg/m2ondays1–5.Thenumberofcoursesvariedfrom1to5.Results:Twenty-ninecoursesofchemotherapywithTpwerecarriedoutin14pts.Completeandpartialresponseswereseenin3ptswithRMS,in1ptwithsynovialsarcomaandin1ptwithmesotheliomaoftheperitoneum.Aminimalresponseincludingreducedpainandbonemarrowremissionwasseenin4ptswithneuroblastoma.Thetoxicityofchemotherapywaslimitedprincipallytothehematopoieticsystem(51.7%),withgrade3–4neutropenia,andgrade2–4thrombocytopenia.Non-hematologicalcomplicationsincludedstomatitis(14.2%)anddiarrhea(14.2%).Currently1patientwithERMSremainsincompleteremissionduringthe41stmonth,onepatientreceivessecond-linechemotherapy,and12havediedduetodiseaseprogression.Conclusions:ChemotherapywithTpcanbeeffectiveagainstRMSandneuroblastoma,andisassociatedwithrelativelylowtoxicity.However,wedidn’tachieveamarkedimprovementofsurvivalinthisgroupofpts.
