Introduction:158patientswithnewlydiagnosedALLweremonitoredinthechildren'shematologydepartmentoftheKirovInstituteofHematologyandBloodTransfusionintheperiodfrom1991to2009.Thirty-fourchildren(21%)experiencedrelapsesatdifferentstagesofobservation.Patientcharacteristicsandresults:Theageofpatientsrangedfrom2to18years(median7.5years),andtheratioofgirlsandboyswas12/22.Fourteen(41%)oftherelapseswereclassifiedasveryearly,5(14.7%)asearly,and15(4%)aslate.Thestructureofrecurrenceswasasfollows:isolatedbonemarrowrelapse,23cases,combined,4,andisolatedextramedullary,7.Inthefirstacuteperiod24(70%)patientsweretreatedwiththeALL–BFM-90protocol,5(14.7%)withALL–MB-2002,and5(14.7%)withotherprotocols.Anti-recurrencetherapywasconductedin30patients;theremainingfourdidnotreceivetreatmentduetoparentrefusal.Twenty-twopatientsweretreatedwiththeALL-REZ–BFM-90protocol,4patientswithALL-REZ–BFM-2002,13patientswithAmericanprotocol–3,and1patientwiththeHeltserprotocol.Thesecondcompleteremission(PR)wasobtainedin60%ofcases(18patients).Arefractoryresulttotherapywasrecordedin3(10%),patientswithearlyrelapse.Nine(30%)patientsdiedofvariouscomplicationsandprogressionofthediseasebeforereachingremission.Ofthe18patientswiththesecondPR,1patientdiedofvariouscomplicationsduringremission,8developedasecondrelapse(3patientsarealiveinone-thirdcompleteremission,andathirdrelapsewasrecordedin1patient,whichendedlethally).Eleven(36%)patientsareinlongremission.Conclusion:TheoverallsurvivalofchildrenwithrecurrentALLattheir18-yearfollow-upwas0.28±0.09,withamedianobservationtimeof52months.
