Awareness and Acceptability of Pre-Exposure Prophylaxis Among Men Who Have Sex With Men In Baltimore

【 摘 要 】

Objective: Men who have sex with men (MSM) are at high risk for HIV in Baltimore City. While pre-exposure prophylaxis (PrEP) has been proven to be safe and effective in preventing HIV among MSM, knowledge, use and acceptability of the method remain low in the United States. This study examined the characteristics associated with awareness of and willingness to take PrEP among Baltimore MSM in 2011.Methods: The Baltimore arm of the National HIV Behavioral Surveillance System (NHBS), or the BESURE Study, a cross-sectional behavioral survey, which used venue- based sampling among adult men at MSM-identified locations. NHBS respondents included in the analysis were MSM over 18 years of age who self-reported as HIV- negative and reported at least one male sex partner in the past year. Logistic regression analysis was utilized to identify characteristics associated with PrEP knowledge and acceptability.Results: Overall, among the 399 eligible men included, 11% (n=44) had heard of PrEP, 48% (n=191) were willing to accept it, and 0% had previously used it. In multivariable analysis, black race (AOR 0.33, CI 0.15 – 0.74) and perceived discrimination against those with HIV (AOR 0.47, CI 0.06 – 0.81) were significantly associated with decreased awareness and perceived discrimination (AOR 1.60, CI 1.01 - 2.51) was significantly associated with increased acceptability of PrEP.Conclusions: Awareness of and willingness to engage in PrEP among Baltimore MSM was modest and associated with black race and perceiving discrimination against someone with HIV. Results indicate a need for further education about the potential utility of PrEP in addition to other prevention methods among high-risk MSM communities. To reduce HIV infection, future interventions need to consider the link between discrimination and potential PrEP use, especially among men of color, and health care providers should increase efforts to provide tailored counseling to decrease the stigma surrounding HIV.
