The Russian Federation has beenconfronting a major epidemiological crisis, which will haveconsiderable human and economic costs if it is not swiftlyaddressed. Between 1999 and 2000, the country experiencedthe world's biggest increase in new cases of HIV/AIDSinfection. The TB epidemic is at risk of becominguncontrollable if the country experiences a moderateepidemic of HIV/AIDS. This report represents the analysisand recommendations of a group of experts from the WorldHealth Organization and the World Bank, and focuses on thecurrent epidemiological situation in the Russian Federationand the existing institutional arrangements available totackle both epidemics. It encompasses projections for the TBepidemic, the interaction with the HIV/AIDS epidemic, andthe impact of a spiraling duel epidemic. The report has foursections. The first briefly describes socioeconomicconditions. The second section discusses the impact on thepopulation of the epidemic, incidence and prevalence in thegeneral and prison populations, diagnosis and treatmentprocesses, and current TB management and control structures.The third section profiles the HIV/AIDS epidemic and itsimpact, and the role of STIs in fostering the disease.Finally, the fourth section describes a mathematical modelof health improvements possible under various TB controlstrategies with implications for HIV/aids as well.
