Any organization can be good atdiscussing its successes and good practices. But can it be atruly effective agent for learning and change if success isall it discusses? Doesn't continual learning requirebeing open about mistakes having the courage to talk aboutthem so they can be corrected and not repeated? Anorganization can be more effective if its staff takesindividual responsibility for creating a culture in whichrisk taking is not only tolerated but also encouraged, andfailure is openly discussed. Some experts ascribe about 80percent of the barriers to an organization's success toissues associated with organizational culture the way itdoes business, including how it learns from its mistakes andfailures. Organizations that openly discuss mistakes andshortcomings tend to get better, become more innovative, andhave greater impact. The 2011 Human Development Forumfeatured a FAILFaire type session, 'how learning fromfailure can make project success more likely.' Thissmart lesson grew out of staff requests for follow-upinformation from that session.
