【 摘 要 】

The objective of this report is toprovide guidance on the design and implementation of servicedelivery models that promote integration of care.Specifically, the report seeks to (i) systematically reviewthe main reform levers of integrated care; (ii) identifyprocesses needed to create an enabling environment for theimplementation of integrated care; and (iii) provideguidance on sequencing the different levers and supportstrategies. This report builds on the findings of recentwork that systematically analyzes well-functioningintegrated programs in the European Union, North America,and elsewhere, as well as five case studies in Croatia andPoland that were carried out as part of this report.
