This activity, Catching-up Regions 3(CuR3), is designed as a one-year engagement with threeselected regions in Poland, based on their regional needsand priorities. The activities of this initiative build onthe results of the CuR Initiative delivered by the WorldBank and the European Commission over the last two yearsacross Poland. The objective of this task is to propose aservice delivery model that will facilitate care for theelderly over the age of 65 years of age in Grudziadz. Thiseffort includes care, services, investments, and newinitiatives in a way that strives to prevent theinstitutionalization of the elderly, as long as possible.The local system of care delivery for seniors does not standalone, and will be molded with consideration to the broadercontext of policy and governance arrangements for health andsocial care already established in Poland, together with thefunding, capacity, and systems that are currently in place.All local and national stakeholders from Ministries tonon-governmental organizations or local community groups canhave a part in the model implementation. The model builds onthe national system’s focus on the changes and adjustmentsthat need to be made at the local level.
