【 摘 要 】

Thalamic hemorrhage is one of deep-seating cerebral lesions. Conservative therapy is preferred to patients with minor thalamic hemorrhage, and external ventricular drainage is usually applied in patients with cerebral ventricular hemorrhage. However, the prognosis of some patients is poor. This study intends to explore that minimally invasive puncture to remove hematoma in patients with the thalamic hemorrhage volume less than 15 ml with disturbance of consciousness (especially the expansion of the hematoma to the hypothalamus or midbrain) is an alternative method. In clinical practice, a wedge-shaped sign of thalamic hemorrhage could be observed in CT images in coronal plane, which was featured by the compression of hematoma on the third ventricle and the aqueduct of midbrain shaped like a wedge into the mesencephalon. The obstruction of cerebrospinal fluid circulation was then evaluated. For persistent hematoma oppression, the precise positioning of stereotactic instrument could be applied to rapidly and effectively remove the oppression, thus improving life quality of affected patients.
