Assessment of Need and Access to Physician and Hospital Beds: A Cross Sectional Province Based Study in Iran

【 摘 要 】

Background:Issuesrelatedtodistributionofhealthresourcesareapersistentpolicyconcernworldwide.ThecurrentstudyaimedtoexamineinequalityindistributionofphysiciansandhospitalbedsinIranbetween2001and2011.Objectives:TheaimofthisstudywasexaminationofthestatusofaccessandneedtophysiciansandhospitalbedsacrossprovincesofIranbetween2001and2011.PatientsandMethods:Thiswasadescriptiveandanalyticalstudy,whichinvestigatedinequalityindistributionofphysiciansandhospitalbedsamongprovincesofIranbetween2001and2011.Inequalitymeasures(includingLorenzcurve,Ginicoefficient,ConcentrationindexandRobinHoodindex)wereusedtoevaluatethelevelofinequalityindistributionofphysiciansandhospitalbeds.Thenumberofhospitalizedpatientswasalsoconsideredashealthneedindex.Dataaboutthenumberofphysicians,hospitalbedsandnumberofhospitalizedpatientswereobtainedfromtheIranianStatisticalCenterandMinistryofHealth.ThedataanalysiswasperformedbySTATAV.12andExcelSoftware.Results:TheGinicoefficientforphysiciansbasedonpopulationlevelandhealthneedindexin2001(2011)were0.19(0.16)and0.48(0.47),respectively.Moreover,theGinicoefficientforhospitalbedsbasedonpopulationlevelandhealthneedindexin2001(2011)were0.16(0.13)and0.38(0.37),respectively.TheconcentrationandRobinHoodindexesforphysicianswere-0.11and14%in2001,whiletheywere-0.13and11%in2011,respectively.Inaddition,theconcentrationandRobinHoodindexesforhospitalbedsin2001were-0.11and7%,whilewere-0.10and5%in2011,respectively.Conclusions:Althoughthetotalnumberofphysiciansandhospitalbedsincreasedfrom2001to2011,butdidnotnecessarilyleadtomoreequalgeographicaldistributionofphysiciansandhospitalbeds.Moreover,ourstudyimpliedthatthehealthresourcesinIranhavebeendistributedaccordingtothepopulationlevel.
