What is the Difference between Causes of ESRD in Iran and Developing Countries?

【 摘 要 】

Introduction:Endstagerenaldisease(ESRD)isoneofthemostcommonlife-threateningdiseases.Inthepasttwodecades,therehavebeensignificantchangesinthecauseofESRDaroundtheworld,andaimofthestudywastodetermineitscausesinKhouzestanprovince,Iran.MaterialsandMethods:WeretrospectivelyreviewedthemedicalrecordsofourESRDpatientsfromJanuary1999toMarch2010.ESRDwasdefinedaschronicandirreversiblelossofkidneyfunctionrequiringdialysis.WeincludedonlyadultESRDpatientsonmaintenancehemodialysis,morethan2monthsbeforeenteringthestudy.SPSS(version15)softwarewasusedfordataanalysis.Results:Inourstudy,atotalof1000adultESRDpatients,withmeanageof51.54±16.39yearswereincluded.Themaletofemaleratiowas1.3:1whichhadnosignificantchangesduringtheperiodofstudy.Themeanageofourpatientsatstartingofhemodialysiswas41.23yearsin1999,butitincreasedto56.91yearsin2010,whichisup15.68yearsduringthisperiod.Inoverall,themostcommoncausesofESRDwereDiabetesMellitus(n=282,28.2%),Hypertension(n=265,26.5%),unknown(n=242,24.2%),Glomerulonephritis(n=79,7.9%),ObstructiveUropathy(n=70,7%),CysticKidneyDisease(n=51,5.1%)andunirenal(n=1,0.1%).Although,twomaincausesofESRDinpatientsaged40yearsandolder(n=761)werealsoDiabetesMellitus(n=255,33.5%)andHypertension,however,inpatientswithlessthan40yearsofage(n=239),themostcommoncauseswereunknownandGlomerulonephritis.TherewerenosignificantdifferenceinthemaincausesofESRD(DiabetesMellitusandHypertension)betweenmenandwomen(P=0.26andP=0.48).Buttherewasasignificantassociationbetweenmeanageofdiabetic(56.71±13.35)versusnondiabetic(49.51±17.03)hemodialysispatients(Plt;0.001).Conclusions:Basedonourfindings,themostcommoncausesofESRDinKhuzestanprovince,IranwereDiabetesMellitusandHypertensionsimilartodevelopingcountries.But,itscausesareunknowninthesignificantpercentofpatientsincontrasttothesecountries.
