Treatment results of children with osteogenic sarcoma according to protocols PE-COSS, COSS-96 and COSS-EURAM0S 1

【 摘 要 】

From1991untilthepresent27patients(pts)withosteogenicsarcomaweretreatedatthedepartmentofpediatriconcology.Inallcasesdiagnosiswasconfirmedbyhistology.Patientagerangedfrom6to17years,median12.5y.o.Sexdistribution:f/m=1/1.5.TheLanskyscorerangedfrom70–80.NineteenptshadprotocoltreatmentaccordingtoPE-COSS,COSS-96andCOSS-EURAMOS.Theremaining8ptsweretreatedrandomly.Mediantimeoffollowupatpresentis71months(max=217mo,min=7mo).Tumorresponseonneoadjuvantchemotherapywasevaluatedin17ptsaccordingtotheWienclassification(Salter-Kuntschi)aftersurgery.TenptsweretreatedaccordingtothePE-COSSprotocol(withoutahighdoseofmetotrexate),8ptsweretreatedaccordingtoCOSS,and1withtheCOSS-EURAMOSprograms,whichincludehigh-doseMTX(12g/m2).InthePE-COSSgroupofptsagoodresponse(I–II)wasachievedin5pts(50%).Overallsurvivalinthisgroupis70%(7outof10alive).IntheCOSSandCOSS-EURAMOSgroup,agoodresponsewasregisteredin4outof8pts(57%),however,inonecaseitwasnotpossibletoevaluatesincethegirlhadsurgeryasafirsttreatmentoptioninanothermedicalinstitution.Overallsurvivalinthisgroupis87.5%(7out8ptsalive).Conclusion:chemotherapyregimensconsistingofhighdosesofMTXprovideamoreremarkabletumorresponseontreatment,whichreflectspositivelyontheoverallsurvivalofptswithosteosarcoma.
