Background:nbsp;Toevaluatetheeffectivenessandtoxicityof2therapyregimensforchildrenandadolescentswithmatureB-NHLusedoverthelast10yearsinspecializedclinicsofhematology/oncologyintheRussianFederation(St.Petersburg,Ekaterinburg,N.Novgorod,Balashikha,Makhachkala,Chelyabisk,Orenburg,Kirov,Rostov-Don,Cheboksari,Novokuznetsk,Arkhangelsk,Perm,Astrakhan,Jaroslavl,Rjazan,Blagoveschensk,andMoscow,Ufa).OneregimenisbasedonoriginalandmodifiedB-NHL90and95.TheotherisaregimencombiningrituximabandlessintensiveCTwithadecreaseddoseofMtx(protocolB-NHL2004mab).Objectives:ToproposeastandardizedprogramoftreatmentforchildrenandadolescentswithmatureB-NHLwhichcouldbeeffectivelyusedinthemajorityofpediatricspecializedhospitalsoftheRussianFederation(RF.)Methods:Adatabasethatincludedinformationfrom28pediatricdepartmentsofhematology/oncologyfollowedbystandardstatisticalanalysis.Results:FromJanuary2004untilApril2010,233untreatedpatientswithmatureB-NHL(BL/B-ALL,143/40pts;DLBCL,66pts)wereenrolledinatrial:m:f=3:1,withamedianageof9.5years.132ptsreceivedtheB-NHL-2004mprotocol,101ptsreceivedNHL-BFM-90or95.Withamed.FUof38mo.(3–70),CRwasachievedin92and82%ofpts,RFSwas0.95and0.91;andEFSandOSwere0.86±0.04and0.72±0.06(p=lt;0.05),respectively.ToxicdeathswerethemaincauseoffailureintheBFM-orientedtypeoftreatment.Conclusions:RituximabcombinedwithconventionalCTisawell-toleratedregimen,associatedwithexcellentresults.Thistherapycanberecommendedasthefirst-linetreatmentforpediatricmatureB-NHL/B-ALL.FuturestudiesarerequiredtodeterminetheoptimaldoseofchemotherapyandrituximabforagressiveNHLformaximumeffectivenesswithminimalacuteandlatetoxicity.nbsp;nbsp;nbsp;nbsp;nbsp;nbsp;nbsp;nbsp;nbsp;nbsp;nbsp;nbsp;nbsp;nbsp;
