Backgroundandobjectives:Mutansstreptococci(MS)areoneofthemajormicrobiologicaldeterminantsofdentalcaries.TheobjectivesofthisstudyaretoidentifydistinctMSandnon-MSstreptococcistrainsthatarelocatedatcarioussitesandnon-cariousenamelsurfacesinchildrenwithsevereearlychildhoodcaries(S-ECC),andassessifcariogenicMSandnon-cariogenicstreptococcimightindependentlyexistasprimarybacterialstrainsondistinctsiteswithinthedentitionofindividualchildren.Design:Dentalplaquefromchildren(N=20;aged3–6)withS-ECCwascollectedfromcariouslesions(CLs),whitespotlesions(WSLs)andnon-cariousenamelsurfaces.Streptococcalisolates(N=10–20)fromeachsiteweresubjectedtopolymerasechainreaction(PCR)toidentifyMS,andarbitrarilyprimed-PCRforassignmentofgeneticstrains.Primarystrainswereidentifiedas≥50%ofthetotalisolatessurveyedatanysite.Inseveralcases,strainswerecharacterizedforacidurityusingATP-drivenbioluminescenceandsubjectedtoPCR-determinationofpotentialMSvirulenceproducts.Identificationofnon-MSwasdeterminedby16SrRNAgenesequencing.Results:Sixty-fourindependentMSornon-MSstreptococcalstrainswereidentified.Allchildrencontained1–6strains.Inmanypatients(N=11),singleprimaryMSstrainswereidentifiedthroughoutthedentition.Inotherpatients(N=4),primaryMSstrainswereidentifiedwithinCLsthatweredistinctfromprimarystrainsfoundonenamel.StreptococcusgordoniistrainswereidentifiedasprimarystrainsonenamelorWSLsinfourchildren,andingeneralwerelessaciduricthanMSstrains.Conclusions:ManychildrenwithS-ECCcontainedonlyasingleprimaryMSstrainthatwaspresentinbothcariousandnon-carioussites.Insomecases,MSandnon-cariogenicS.gordoniistrainswerefoundtoindependentlyexistasdominantstrainsatdifferentlocationswithinthedentitionofindividualchildren,andtheaciduricpotentialofthesestrainsmayinfluencesusceptibilityinthedevelopmentofCLs.Keywords:oralstreptococci;distributionofgenotypicmutans;streptococcalstrains;Streptococcusmutans;Streptococcusgordonii;severeearlychildhoodcaries(Published:29October2014)Citation:JournalofOralMicrobiology2014,6:25805-http://dx.doi.org/10.3402/jom.v6.25805
