ItisbroadlyacceptedthatHIVDNAinlymphoidandmyeloidcellspersistsdespitecombinationantiretroviraltherapy.RecognizedastheAchillesheeltoHIVeradication,theroleoftheseperipheralreservoirsinHIVmorbidityislesswelldeveloped.TheburdenofHIVDNAinperipheralmononuclearcellsislinkedtoHIVdiseaseoutcomessuchastimetoAIDSdiagnosis,survival,andCD4T‐lymphocytecounts.MonocytesareaminorHIVDNAreservoir,andtheburdenofHIVDNAinthesecellsappearstobelinkedtodementia,suggestingthatresidualinfectioninthissubsetislinkedtotissue‐relatedHIVcomplications.Sincemonocytesarelikelyinvolvedintraffickingvirustothebrain,thereisastrongmechanisticlinkunderlyingthisdiscovery.Herein,wesummarizeourcurrentunderstandingofmonocyteHIVDNAandcentralnervoussystemdysfunctioninhumans.Wepresentamodeltounderstandtheserelationshipsandsuggestpossibletreatmentapproachestobetested...【 授权许可】
