目标管理对老龄食管癌开胸手术患者呼吸功能锻炼依从性的影响

【 摘 要 】

Abstract：Objective To investigate the effect of management by objectives on compliance of respiratory function training in elderly esophageal cancer patients with open chest surgery.Methods A total of 32 elderly esophageal cancer patients with open chest surgery were enrolled and assigned to the control group(n=17)and the study group(n=15). The control group received routine nursing care and the study group was given nursing management by objectives, in which goal setting, training enhancement, objective establishment and effect evaluation were carried out by management group. The compliance of respiratory function training, incidence of pulmonary complications, length of stay and hospitalization cost were compared between two groups.Results There were 12 patients(80.0%)with high compliance of respiratory in the study group and 6 patients(35.3%)with high compliance of respiratory function training in the control group, with a significant difference(P<0.01). Patients in the study group had shorter length of stay and lower hospitalization cost compared with those in the control group(P<0.05). There was no significant difference in incidence of postoperative pulmonary atelectasis and Pneumonia between two groups(P>0.05).Conclusion Management by objectives is an effective tool to improve the compliance of respiratory function training and postoperative rehabilitation in elderly esophageal cancer patients with open chest surgery.
