Thcellshavelongbeenrecognizedasvitalcomponentsoftheadaptiveimmunesystem.Untilrecently,CD3+CD4+Thcellsweredividedintocell‐mediatedTh1orhumoralTh2responses.However,theTh1‐Th2hypothesisfailedtoaccommodatethemorerecentlydescribedTh17cells.Today,themajorThcellsubsetsincludeTh1,Th2,Th9,Th17,Th22,andTregs,eachofwhichproducespecificeffectorcytokinesunderuniquetranscriptionalregulation.Specifically,Th17cellsproduceeffectorcytokinesIL‐17,IL‐21,andIL‐22undertheregulationofROR‐γt.Th17lymphocyteswerefirstdescribedasorchestratorsofneutrophilrecruitmentandactivationandaskeyplayersinchronicinflammationandautoimmunity.MorerecentevidencesuggestthatTh17lymphocytesandtheireffectorcytokinesplayacrucialroleinmaintainingmucosalimmunityandbarrierintegrity,includingtheskin,lung,andgut.Burninjuryinducesglobalchangestothesystemicimmuneresponse,includingsuppressedimmunefunctionandincreasedsusceptibilitytoinfection.Moreover,burntraumaisassociatedwithremoteorganinjury.Thisrelationshipbetweenburnandremoteorganinjurysupportsthehypothesisthatimmunesuppressionmayfacilitatethedevelopmentofsepsis,systemicinflammatoryresponsesyndrome,andmultipleorgandysfunctionsyndromeincriticallyillburnpatients.Herein,wediscussthisemergingadaptivecellsubsetincriticalcaresettings,includingburninjuryandclinicalsepsis,andhighlightthepotentialtherapeuticroleofIL‐22...
