【 摘 要 】
This paper assesses the extent to whichprovider payment mechanisms can help developing countriesaddress their leading health care problems. It firstidentifies four key problems in the health care systems indeveloping countries: 1) public facilities, which providethe bulk of secondary and tertiary health care services inmost countries, offer services of poor quality; 2) providerscannot be enticed to rural and urban marginal areas, leavinglarge segments of the population without adequate access tohealth care; 3) the composition of health services offeredand consumed is sub-optimal; and 4) coordination in thedelivery of care, including referrals, second opinions, andteamwork, is inadequate. The paper examines each problem inturn and assesses the extent to which changes in providerpayments might address it.【 预 览 】
