This is a companion volume to TR-475-HLTH, Redesign of the National Hospital Discharge Survey: Conceptual Framework and Feasibility Study Final Report, which reports the development of an approach for redesigning the National Hospital Discharge Survey and identifying, through a feasibility study, specific data elements to be included and field procedures to be used in that survey. It offers the instructions to the hospitals that were used for the feasibility study. The feasibility study informed the modifications that were subsequently made to the forms and procedures in this volume. The modified forms and procedures are included as Appendix A in the main report.
