【 摘 要 】
Patient safety as a basic principle of quality isdominant in the global health agenda andmentioned as an essential requirement to organize aqualified health care system.1 In hospitals, the Director orchief executive officer (CEO) has the main responsibilityfor patient safety in the whole hospital, although all clinicalleaders are responsible for patient safety within their ward.To clarify responsibilities and support clinical leadersin their work with patient safety, a formal structure isnecessary. This formal structure may include a directorof patient safety and patient safety officers (PSOs) in eachlarge clinical unit. Moreover, a patient safety committeemust be established to reviews all serious events and takesinitiatives to improve clinical practice. Unfortunately, the main cause of medical errors in most cases are dueto system weakness and therefore the Director and otherclinical leaders have responsibility as other clinical staffswhen medical errors happen.
