Alternatives to Acellular Dermal Matrix in Breast Reconstruction: Outcomes from Dermal Autograft Assisted Tissue Expander Breast Reconstruction in 104 Consecutive Patients

PURPOSE: Acellular dermal matrices (ADMs) have manybenefits when used in implant-based breast reconstruction, but they add cost to the procedure and may increase complications. In addition, many patients prefer to avoid humanbased products if there is an autologous option. For thisreason, surgeons have sought viable alternatives to ADM inbreast reconstruction. Dermal autografts can be harvested atthe time of mastectomy and represent a useful, autologousalternative to ADMs.METHODS: One hundred four consecutive patients (171breasts) underwent breast reconstruction using tissueexpanders and dermal autograft over a 7-year period. Ageranged from 32 to 73 years (median, 53 years). Autograftswere harvested from either the lower abdomen (n = 86) orthe inferior breast (n = 18). Dermal autografts were usedto cover the inferior pole of the tissue expander at the timeof expander placement. Demographic data, clinical history,and harvest time were recorded. The initial fill volume,number of expansions, cost, and complications were compared to ADM-assisted reconstruction. Breast-Q surveyswere mailed to all patients, and satisfaction ratings werecompared with historical values of patients undergoingADM-assisted breast reconstruction. A cost analysis wasperformed of dermal autograft assisted reconstruction, andthis was compared with historical values for ADM-assistedreconstruction.
