Associate Editor's Introduction—There is an increasing trend toward the use of tissue valves in the aortic position in younger patients, either as a percutaneous option using the transcatheter approach (ie, transcatheter aortic valve replacement) or as a potential “first-stage” with a bioprosthetic valve and an eventual valve-in-valve transcatheter aortic valve replacement down the road. One argument made is that either of these approaches avoids the need for long-term anticoagulation viz-à-viz mechanical valves.
