【 摘 要 】
An ill-appearing woman in her early 50s presentedto the emergency department with a 12-hour historyof right-sided abdominal pain associated with chills.Her medical and surgical history were significantfor hypertension, a laparoscopic total abdominalhysterectomy, and a laparoscopic cholecystectomy.The patient had not had a previous colonoscopy.She initially attributed the pain to chronic constipation; however, her pain subsequently worsenedand became diffuse throughout the day. On physicalexamination, the patient was afebrile, with normalblood pressure parameters, and slight tachycardiawith a pulse of 101 beats per minute. Laboratoryvalues were within normal limits. Her abdomendemonstrated diffuse tenderness to palpation withsigns of peritonitis. A CT scan was performed andshowed a foci of extraluminal air concerning fora small bowel perforation with an adjacent fluidcollection (figure 1).
