【 摘 要 】
The publication of To Err is Human by theInstitute of Medicine (IOM, now the NationalAcademy of Medicine) in 2000 proved tobe a watershed moment in healthcare.1 Forthe first time, national attention focused onflaws in the healthcare system that regularlyled to patient harm. The report drew widespread attention from the press and began animportant national discussion that continuestoday. A follow-up report from the IOM in2004, Patient Safety Achieving a New Standard,recommended a series of sweeping changes.2Principal among them, the IOM argued that‘all healthcare organisations should establishcomprehensive patient safety programmesoperated by trained personnel…’ The mainaim of the proposed programme was tocreate systems for collecting and analysingpatient safety data and using the knowledgeto reduce risk and prevent future incidents.
