Medical decisions are best made in consultation with high-qualityevidence, but, where such evidence is incomplete, clinicians mustcounsel patients using available knowledge. This is the case forpatients with obstructive sleep apnea (OSA) who lack excessive daytime sleepiness (EDS) and established cardiovascular disease (CVD).In the current issue of SLEEP, Dr. Pengo and colleagues offer a framework for management that contextualizes current evidence [1]. Theauthors propose the three-step OSCAR algorithm where clinicianssequentially address excess weight, assess symptoms, and considercardiovascular risk. While we believe that this algorithm is logicaland helpful, there are nuances that merit consideration.
