【 摘 要 】

Se investigaron pacientes con antecedentes de haber elevado sus cifras tensionales ante situaciones estresantes, pero que habitualmente eran normotensos (hipertensos eventuales). Este tipo de paciente constituye una causa frecuente de suspensión del acto quirúrgico por hipertensión arterial. Se propuso utilizar la prueba de estimulación por frío, ante la necesidad de identificar el comportamiento tensional operatorio de estos pacientes y por su probada sensibilidad y especificidad como prueba de provocación de hipertensión arterial y equivalente al estrés quirúrgico, para formular un pronóstico de comportamiento tensional transoperatorio. Se trabajó sobre una muestra de 30 sujetos, a todos se les realizó la prueba, 24 tuvieron resultados positivos y se les indicó tratamiento individualizado, 6 fueron negativas, no se prescribió tratamiento por no expresar hiperreactividad. No se produjo hipertensión perioperatoria, ni suspensiones; se concluyó que la prueba es un valioso instrumento para la predicción tensional transoperatoria y auxiliar en la desición de prescribir hipotensores.
Some patients who had previously had elevated arterial tension resulting from stressing situations, but who were usually normotensive (occasional hypertensives) were studied. This type of patient is the most frequent cause of suspension of the surgical procedure as a result of arterial hypertension. It was proposed to use the cold pressure test due to the need of identifying the tensional operative behaviour among these patients and to its sensitivity and specificity capable of producing an arterial hypertension similar to the one caused by surgical stress. It allows to have a prognosis of the tensional transoperative behavior. 30 individuals were included in the sample and all of them underwent this test. 24 had positive results and were indicated individualized treatment. The other 6 were negative and no treatment was prescribed since no hyperreactivity was observed. There was neither perioperative hypertension nor suspensions. It was concluded that the test is a valuable tool for tensional transoperative prediction and that it is also useful at the time of prescribing hypotensors.
