Endoscopic transcanal excision of a rare huge middle ear capillary hemangioma – Case report and literature review

【 摘 要 】

Objectives: We describe a rare case of huge middle ear capillary hemangioma extending to the attic, which was entirely removed by the endoscopic transcanal approach. Case report: A 40-year old female presented with a long-standing history of right-sided, intermittent mild to severe otalgia with mild hearing loss. Examination revealed reddish, non-pulsatile, retrotympanic mass occupying complete mesotympanum. High-resolution CT scan and MRI revealed soft tissue mass occupying mesotympanum and epitympanum with differential diagnosis of glomus tympanicum. Total endoscopic transcanal excision of the mass was done under general anesthesia followed by augmented type III tragal cartilage palisades tympanoplasty. Formal histopathological examination revealed middle ear capillary hemangioma. There were no postoperative complications noted. Conclusion: The middle ear capillary hemangioma is a very rare entity and should be considered as one of the differential diagnoses of vascular tumors of the temporal bone. Transcanal endoscopic excision of the huge middle ear mass can be done, achieving excellent results and avoiding postaural incision, tissue retraction, and unnecessary mastoidectomy, making it minimally invasive surgery.
