【 摘 要 】
ObjectiveTo investigate the effect of specialist nurse-led continuing care on self-management skills and blood-glucose control in elderly diabetic patients. MethodsTotally 80 elderly patients with type 2 diabetic mellitus were randomly divided into the study group and control group, with 40 cases in each group. All patients received community-based chronic disease management during follow-up, and those in the study group received the specialist nurse-led continuing care. The levels of HbA1c, Fasting Plasma Glucose (FPG), 2-hour postprandial blood glucose(2hPG), and scores of the Self-efficacy for Diabetes(SED) and Summary of Diabetes Self-Care Activities Questionnaire (SDSCA) were compared between two groups after a 6-month follow-up. ResultsAt 6-month follow-up, a better improvement in HbA1c, FPG and 2hPG were found in the study group compared with those in the control group (P<0. 05). Scores of SED and SDSCA in study group were higher than those of control group at 6-month follow-up (P<0. 01). ConclusionSpecialist nurse-led continuing care is effective to enhance the self-management skills and blood-glucose control efficacy in elderly diabetic patients.
