The Effects of Lidocaine and Mefenamic Acid on Post-Episiotomy Pain: A Comparative Study

【 摘 要 】

Background:Mostwomensufferpainfollowinganepisiotomyandoralnon-steroidalanti-inflammatorydrugsarecommonlyusedforpainrelief.Duetothegastrointestinalsideeffectsoforaldrugs,itseemsthatwomenaremoreacceptingoftopicalmedicationsforpainrelief.Objectives:Therefore,theaimofthisstudywastocomparetheeffectsoflidocaineandmefenamicacidonpost-episiotomypain.PatientsandMethods:Thisclinicaltrialwascarriedoutin2011.Itinvolvedsixtywomenwithsingletonpregnancywhoweregivenanepisiotomyat38to42weeksofgestation.Theparticipantswererandomlydividedintotwogroups.Onegroupreceived2%lidocainecream(n=30),whiletheothergroupreceived250mgofmefenamicacid(n=30).Thedatawerecollectedviaaquestionnaireandavisualanalogscale.Painintensitywascomparedfromthefirstcomplaintbythemotherandat6,12,and24hoursafterthedeliveryinbothgroups.ThedatawereanalyzedusingSPSS(version16),thet-test,andthepairedt-test,andaPvalueoflessthan0.05wasconsideredsignificant.Results:Themeanintensityofpainatthefirstcompliantwas4.92±1.9inthelidocainegroupand4.90±1.5inthemefenamicacidgroup,andthedifferencewasnotstatisticallysignificant(P=0.20).Additionally,therewasnosignificantdifferenceinthemeanintensityofpost-episiotomypainbetweenthetwogroupsat6(P=0.05),12(P=0.36),and24(P=0.98)hoursafterchildbirth.Conclusions:Theeffectsofthelidocainecreamandmefenamicacidweresimilarintermsofthereliefofpost-episiotomypain.Lidocainecreamthereforerepresentsagoodalternativetomefenamicacid,whichiscommonlyusedtoreducepainfollowinganepisiotomy,especiallyinwomenwhoarebreastfeedingandwhowishtoavoidoralanalgesicdrugsbeingsecretedintheirmilk.
