【 摘 要 】
Background:Preeclampsiaisahypertensivedisorderofpregnancy,whichisassociatedwithincreasedmaternalandprenatalmorbidityandmortality.Oxidativestressassociatedwithpreeclampsiamaybeaconsequenceofreducedantioxidantdefensepathwaysthatmightinvolveinadequateglutathioneperoxidase(GPx)levels,perhapslinkedtoreducedseleniumavailability.ThesolubleFMS-liketyrosinekinase-1(sFlt-1)thatcontributestoendothelialdysfunctionmaybepartiallyresponsiblefortheclinicalmanifestationofpreeclampsia.Furthermore,elevatedplasminogenactivatorinhibitor-1(PAI-1)anddecreasedplasminogenactivatorinhibitor-2(PAI-2)arefoundinpreeclampticwomen.Hence,thePAI1:PAI2ratiomaybeapredictorofpreeclampsia.Objectives:TheobjectiveofthisstudywastoevaluatetheeffectsofseleniumsupplementationonsFlt-1,GPxactivityandPAI1:PAI2ratioinpregnantwomen.MaterialsandMethods:Atotalof125high-riskpregnantwomen(withafamilialhistoryofhypertension,hyperlipidemiaandotherriskfactorsforpreeclampsia)inthefirsttrimestersofpregnancywereassignedtoeitherselenium(n=61)orplacebo(n=64)groups.Theseleniumgroupreceived100μg/dayofseleniumasaselenium-yeasttabletforsixmonths.Theplacebogroupreceivedaplaceboyeasttabletforthesameperiod.Atthebeginningofthetrialandattheend,bloodsampleswerecollectedandthelevelsofsFlt-1,PAI-1,PAI2andGPxweremeasuredinbloodserumandplasma.Results:Serumseleniumconcentrationswereraisedintheseleniumgroup(Plt;0.001)fromthefirsttothethirdtrimester,butwasunchangedintheplacebogroup(P=0.85).TheresultsshowedthatsFlt-1hadsignificantlyincreasedinbothgroupsbytheendofthegestationperiod,andseleniumsupplementationhadnosignificanteffectontheseleniumgroup(P=0.51).However,GPxactivitywassignificantlyincreasedintheseleniumtreatmentgroupaftersupplementationcomparedtothecontrolgroup(Plt;0.001).ThePAI1:PAI2ratiowasnotsignificantlydifferentbetweenthetwogroups(P=0.44).Conclusions:SeleniumintakeduringthesecondandthirdtrimesterofpregnancyincreasedGPxactivitybutdidnothaveasignificanteffectonserumsFlt-1levelsortheratioofPAI1:PAI2intheserum.
