Transdermal Nitroglycerin Versus Oral Nifedipine for Suppression of Preterm Labor

【 摘 要 】

Background:Oneofthemostseriouscomplicationsofpregnancyispretermlabor(PTL)anddelivery.Further,therearedifferenttocolyticagentsavailabletoenhancethetimeofdelivery.Objectives:Theaimofthisstudywastocomparetheeffectivenessoftransdermalnitroglycerin(glyceryltrinitrate,GTN)andoralnifedipineformanagingpretermlabor.PatientsandMethods:ThiswasarandomizedclinicaltrialinvolvingwomenwhowereadmittedtoourhospitalwithadiagnosisofPTL.GrouponereceivedtransdermalGTN,whereasgrouptworeceivedoralnifedipine.Allvitalsigns,fetalheartrate(FHR),contractions,dilation,andeffacement,aswellasgestationageatthetimeofdelivery,havebeenmonitoredandevaluatedinbothgroupsofpatients.Ourmaingoalwasthepostponementofdeliverysoastosecurethemostbeneficialeffectofprimarycorticosteroidadministrationforthefetus.Results:Bothgroupswerematchedregardingage,gestationalageatthetimeoftocolytictherapy,heightoffundus,historyofpregnancy,abortion,delivery,andstillbirth,primarybodytemperature,andsystolicanddiastolicbloodpressure.ThosewhoreceivedGTNhadamoreprolongeddelayofdelivery.Also,theGTNgrouphadinfantswithbetterone-andfive-minuteApgarscores.Althoughbothgroupsreportedminimalsideeffects,tachycardiaandnauseawerereportedmorefrequentlyinGTNpatients.Conclusions:TransdermalGTNwasshowntobemoreeffectiveincontrollingPTLcomparedtooralnifedipineintermsofbothmaternalandneonataleffects.
