Effect of Opium Addiction on Perioperative Needs to Inotropic Agents in Coronary Artery Bypass Surgery: a Case-Control Study

【 摘 要 】

MethodsandMaterial:62malepatientsupto75yrsoldscheduledforelectiveon-pumpCABGwithejectionfraction>40%wereassignedtotwogroupsaccordingtoDSM-IV,TR:Opiumdependentandnondependentpatients.Anesthesiainducedandmaintainedsimilarlyinallpatients.Duringtheperioperativeperiodbloodpressure(BP),heartrate(HR,centralvenouspressure(CVP),cardiacoutput(co),systemicvascularresistance(SVR),typeandlengthofinotropesusedandthefrequencyofintraaorticballoonpump(IABP)insertionweremonitoredandrecorded.Pairedsamplet-test,Independentsamplet-testandChi-Squaretestwereusedtocomparevariablesamongtwogroups.Result:Demographicdatashowednosignificantdifferenceinage,bodymassindex(BMI),preoperativehemoglobin,ejectionfraction(EF)andnumberofgraftsintwogroups.Alsotherewasnosignificantdifferenceregardingtheincidenceofcardiovascularriskfactorsandmeanbloodpressure(MBP).Thefindingsindicatednosignificantdifferenceinthefre-quencyofinotropeusage[23(74.1%)vs.25(80.6%);p=0.381]andfrequencyofIABPinsertion[2(6.4%)vs.3(9.6%);p=0.187]betweentwogroups.Butthelenghtoftheneedtoinotropesinopiumdependentpatientswassignificantlylowerincomparisonwithnonde-pendentpatients(26.8±9.9vs.36.1±17.45;p=0.042).Objective:tocomparetheneedtoinotropicagentsinopiumdependent(OD)andnonde-pendent(NOD)patientsduringtheperioperativeperiodinpatientsundergoingcoronaryarterybypassgraft(CABG)surgery.Conclusion:Althoughthelengthoftheneedtoinotropicsupportinnonopiumdependentwaslongerascomparedtoopiumdependentpatients,frequencyofinotropeneedandspecificinotropeusageandalsofrequencyofIABPinsertionwasnotdifferentbetweenthetwogroups.
