Is life Easier and More Pleasurable after Total Hip Arthroplasty?

【 摘 要 】

Background::Evaluationofoutcomeofsurgerieshasshiftedtoself-evaluation.Healthoutcomeshouldbedefinedforeachcaseandbemeasuredbyvalidandstandardizedtools.TheaimofthisstudyistoevaluatetheeffectofTHAonvariousaspectsofpatients’life,includingtheirsocial,familial,andpsychologicalactivities,aswellastheirphysicalperformance.METHODS:Thisstudyevaluated220patientswhounderwenttotalhiparthroplastybecauseofhipjointosteoarthritisinBaqhiatallahuniversityhospitals,TehranandKashanihospital,Isfahanfrom1998to2008.Controlgroupincluded220patientsofthesameagegroupadmittedinhospitalsandorthopedicclinicsbecauseofhipjointosteoarthritisandhavenotundergonearthroplastyuntilnow.Patientswereevaluatedthoroughlyby“Shortformofgeneralhealthstatus”(SF-36),“TheDiseaseSpecificSymptom(WOMAC)”Questionnaire,the“ArthritisPatientPreferenceDisability”Questionnaire.RESULTS:MeanscoresofSF-36andWomacquestionnaireswere53.07±18.43and63.02±17.42respectivelyforcontrolgroup,whilethedeterminedscoresforcontrolgroupwere81.3±16.31and42.53±18.73,respectively.Thescoresobtainedfromthesetwoquestionnairesshoweda23.3%improvementingeneralhealthstatusandan18.1%improvementinspecificsymptomofthediseaseincasegroup.Thetwogroupshadasignificantdifferenceconsideringthesetwoparameters(P<0.001).CONCLUSIONS:Totalhiparthroplastyincreasedqualityoflife,improvedfunctionandproducedgreatsatisfactioninthemajorityofcasesinoursociety
