Intravenous Infusion of Lidocaine During Anesthesia Decreases Duration of Post Appendectomy Ileus and Systemic Analgesic Consumption

【 摘 要 】

BackgroundandAim:Animalandhumanstudiesshowedthatintravenousinfusionoflidocainemayhavebeneficialeffectsonreducingpainandfasterrecoveryofbowelmovementsduringpostoperativeperiod.Thepresentstudyaimedtoinvestigatetheeffectoflidocaineinfusiononileusafterappendectomy.Methods:Inthisrandomizeddouble-blindedclinicaltrial,40patientsundergoingappendectomyenrolledinthestudy.Theregimentofanesthesiawassimilarinbothgroups.Thelidocainegroupreceived1.5mg/kgbolusdoseinjectionoflidocaineimmediatelybeforeinductionofanesthesiaandafterendotrachealintubationinfusionoflidocainestartedwiththerateof3mg/min(inpatientsweighinglessthan70kg,with2mg/min).Infusionoflidocainewascontinuedduringsurgeryand60minutesafterit.Thecontrolgroupreceivednormalsalineusingthesamemethodanddosageasaplacebo.Afterthesurgery,upto3days,ablindobserverrecordedthepatient’ssystemicanalgesicconsumptionandthetimeoffirstexperiencedpassageofintestinalflatus.Results:Averageofage,durationofanesthesiaandgenderdistributioninbothgroupsshowednosignificantdifference(p>0.05).Averageelapsedtimefromtheendofoperationtothefirstpassageofflatusinthestudygroupandthecontrolgroupwere31.64±5.49and41.31±9.89hrs,respectively(p<0.0001).Themeanpostoperativeanalgesicconsumption(pentazocine)onthefirstdayafteroperationinstudyandcontrolgroupwere18±6.15and27.75±5.49mg,respectively(p<0.0001).Ontheseconddayaftersurgery,23patientswhohadreceivedanalgesics,5individualswereinthestudygroupand18individualswereinthecontrolgroup(p=0.00007).Conclusions:Theresultsofthisstudyshowedthatlidocaineinfusionduringanesthesiaandupto1hourafterappendectomyreducesthedurationofileusandtheneedforsystemicanalgesicpostoperatively.
