Options for preserving reproductive function in patients with Hodgkin’s lymphoma receiving dose-intensive chemotherapy

Theextremeimportanceofpreservingovarianfunctioninwomenattheirreproductiveageisbeyondanydoubt.Unfortunately,theintensiveapplicationofcytostaticregimens,especiallythosecontainingalkylatingagents,isaccompaniedbyalossofovarianactivity.Inrecentyears,somemethodsofpharmacologicalovarianprotection,suchascombiningoralcontraceptivesandLHRHagonistshavebeenwidelyintroduced.However,theefficiencyofthesetherapeuticapproacheshasnotyetbeendefinedforthepatientswithHodgkin'slymphomaundergoingadose-intensiveВЕАСОРР-14treatmentprotocol.Hence,thepurposeofourstudywastomonitorfertility-relatedfunctioninpatientsattheirreproductiveageaftertheirexposuretodose-intensivechemotherapy(BEACOPP-14regimen),accompaniedbypharmacologicalinhibitionoftheirovarianfunction.AstudygroupincludedsixteenpatientswithHodgkin’slymphoma(clinicalstageIIIA–IVB),whosemedianagewas22years(16to26yearsold).AllpatientsunderwenttheBEACOPP-14regimen(8rounds,atthetotaldoseofcyclophosphamideexceeding8g).Menstrualdysfunctionwasnotreportedinanypatientbeforetreatment.Occasionalgynecologicalpathologywasnotedintwocasesbeforetreatment(chronicadnexitiswithoutexacerbations).Fourteenpatientsreceivedacombinationoforalcontraceptives(regulon,repeatedadministrationduringtheentiretreatmentcourse),andtwopatientsweretreatedwithZoladex,ananalogueofnaturalluteinizinghormone-releasinghormone(asingleinjectionevery28days).Examinationofthepatientswasperformedrepeatedly,andtheirhormonalprofilewasevaluatedat3monthsand6monthspost-treatment.Restorationofregularmenstrualfunctionsandhormonalprofilewerenotedinallpatients,bothat3and6months.Accordingtoourresults,pharmacologicalovarianprotectiontechniquesallowedanefficientandhigh-qualitymaintenanceofmenstrualfunctioninpatientswithHodgkin’slymphomasubjectedtointensivechemotherapy.
