Quality of life in patients in prolonged remission from Hodgkin's lymphoma (HL)

HLisapotentiallycurablediseaseformostpatients.ThemodernconceptoftreatmentofchildrenandadolescentswithHL,implementedintheGerman-AustrianprotocolDAL-HD-90,aimsatminimizingthedosesandfieldsofradiationtherapy(RT)andreducingthetotaldosesofalkylatingagentsandanthracyclinesinordertodecreasetheriskoflateevents(secondarytumors,endocrineandcardiovascularcomplications),violationsoffertility,andpreservationofQoLofcuredpatients.TheassessmentofQoLisanimportantmethodofstudyingthepatient’sstatus,asitintegratesphysical,psychological,andsocialwell-beingcharacteristics.ThepurposeofthisworkwastheinvestigationofQoLincuredpatientsinlong-termremissionaftertherapy(inaccordancewithprotocolDAL-HD-90m)incomparisonwithahealthycontrolgroup.Patientsandmethods:Duringtheperiod1995–2007,94patients(m:–37,f:–57)aged15to29(median16.7years)wereenrolledaftertreatmentwitharadiotherapy-modifiednbsp;DAL-HD-90protocol.Health-relatedQoLresearchwasconfinedtothepatientswithoutsignsofprogressionorrelapse,andwithacompleteresponsedurationnbsp;lastinglongerthanfiveyearsatthetimeofthequestionnaire.Thecontrolswerehealthystudentswithnomedicalprofile(n=50;medianage23.4;range18–32years).ThequestionnaireusedwasEORTCQLQ-C30.Results:Completeresponsewasreachedin91%ofpatients.Six-yearoverallsurvivalwas0.93±0.03;andeventfreesurvival,–0.84±0.06.Inthecancersurvivorgroupnbsp;therewasahighfrequencyofnauseaandvomitinganddyspnoea(Fig.1)incomparisonwiththehealthyvolunteers.Ontheotherhand,fatiguewashigherinthehealthyvolunteers,whichwasmostlikelycausedbyahigherlevelofsocialandphysicalactivity.Theglobalhealthstatuswassimilarinbothgroups.Figure1.ComparativeanalysisofQoLofpatientswithprolongedremissioninHodgkin'slymphomaandhealthyvolunteers.*-plt;0,05Conclusions:TherevisedDAL-HD-90protocolprovidesahighQoLinadolescentsandyoungadultscuredofHodgkin’slymphomacomparabletothatintheirhealthycontemporaries.nbsp;nbsp;nbsp;nbsp;nbsp;nbsp;
