Minimal residual disease evaluation in patients with acute lymphoblastic leukemia

Minimalresidualdisease(MRD)isthepersistenceofalownumberofleukemiccellsinpatientswhoachieveclinicalandhematologicalremission.TheaimofthisstudywastodefinetheprognosticvalueofMRDdetectionbyflowcytometry.nbsp;nbsp;Materialsandmethods:Weanalyzed145bonemarrowsamplesfrom41patientswithmultiparametryflowcytometry.21patients(childrenandyoungadults)hadnewlydiagnosedB-precursorsleukemia,treatedwiththechemotherapyprotocol“Moscow-Berlin2008”,and20patientshadundergoneallogeneichematopoieticstemcelltransplantation(alloHSCT).Thefollowingmonoclonalantibodycombinationswereused:CD10/CD34/CD19/CD20,CD22/CD34/CD19/CD45,CD38/CD34/CD19/CD45.ToverifytheirMRD-statuswecomparedtheflowcytometrydatawithmolecularbiology,cytogenetic,andmorphologyassays.Results:AttheendoffirstconsolidationtherapyweobservedapositiveMRDin4patients(19%);threeofthemalsohadapositiveMRDafterthesecondconsolidation,whichthenbecamenegative.nbsp;nbsp;nbsp;nbsp;Fourpatients(19%)hadapositiveMRDstatusafterthesecondconsolidation.Allpatientswereinremissionatthetimeofthestudy(medianofobservation11months).In10patients(50%)post-alloHSCTtheMRDwasnegative,whichcorrespondedtothemolecularbiologydata.ThreepatientshadonepositiveMRD,theyalsostayedinremission.Fourpatients(20%)hadMRD-positivestatusoneverytime-pointofobservation.Threeofthemdevelopedrelapse.Overall4-yearsurvivalwassignificantlyhigher(p=0.014)inpatientswithanegativeMRDstatusoneverytime-point.nbsp;nbsp;nbsp;Conclusion:MeasurementofMRDbymultiparametryflowcytometry,basedonleukemia-associatedimmunophenotype,providesprognosticinformationinchildrenwithALL.
