High dose chemotherapy with PBSC support in pediatric brain tumor patients

TheroleandplaceofHDCTwithHSCsupportinthetreatmentofpediatricbraintumorsremainscontroversial.In2008westartedaprotocol:Thetreatmentofpatientswithbraintumorsundertheageof3yearswithintensivechemotherapy(NeuroIPO).Currently35ptsareincludedinthestudywiththefollowupdiagnosesbeing:ependimoma,12,medulloblastoma,13,CPC,1,ATRT,6,andsPNET,4.Themediumageatdiagnosiswas19.7m.FiveptsreceivedtandemtransplantswithASCrescueafterCBDCA/TMZ/CPMandCBDCA/TMZ/MELregimensasaconsolidation.Threeoutoffiveptsarealiveanddiseasefreeformorethanoneyear.Two-yearEFSforthewholegroupis42%.From2009wehavebeenpilotingaconditioningregimenwherecraniospinalirradiationiscombinedwithmelphalanandtopotecan.ThefirstlevelofRTis10Grey,andshowsnomajorunexpectedtoxicity.Since2009theMoscowneurooncologygrouphasbeenconductingatrailwithintensivechemotherapywithHSCsupportinptswithmedulloblastomawhoaremorethan4yearsofage(modifiedSt.Jude2003protocol).AftersurgeryandconformalRTtotheresidualtumor/ortumorbedptsaretreatedwith4coursesofCT(Cyclo+CDDP+Vcr),eachwithHSCsupport.Todate10ptsareincluded.Totalamountofcoursesdelivered:30.Amediannumberof1.45(0.7–2.7)х106/kgCD34+wasreinfusedaftereachcourseofCT.Thetoxicityoftherapywasmainlyhematological.NoTRMwasregistered.5ptshavefinishedtheirtreatment.NoneoftheptsprogressedonCTandhaverelapsedinthisshortperiodofobservationtodate.
