Evaluation of tyrosine hydroxilase (TH), GD2 and ELAVL4 expression detection for bone marrow (BM) involvement in neuroblastoma (NB) patients

Introduction.Recently,itwasidentifiedthatPHOX2BispresentlythebestsinglemarkerforPCR-baseddetectionofBMlesioninNBpatients(pts).Aim.TostudytheapplicabilityofTH,GD2,andELAVL4expressiondetectionincomparisonwithPHOX2BfortheassessmentofBMinvolvementinNBpatients.Methods.Molecularmarkerexpressionwasevaluatedbyreal-timePCRin183BMsamplesobtainedfrom37NBpts,in16BMsamplesfrom16patientswithoutmalignancies,andin2neuroblastomacelllines(IMR-32,Kelly).ThesampleswereconsideredaspositiveifdetectablePHOX2BexpressionortumorcellswerepresentintheBMsmears.Thethresholdlevel(TL)foreachmarkerwascalculatedbyROCcurveanalysis.Diagnosticsensitivity(DS),specificity(Sp),positiveandnegativepredictivevalues(PPV,NPV),andoverallcorrectprediction(OCP)werecalculated.Results.PHOX2BandTHexpressionwasnotdetectedinnormalBM.GD2andELAVL4expressionwasrevealedin10outof16normalBMsamples.THandPHOX2BROC-curvesdidnotdiffer(p=0.224).THhadhighvaluesofDS,Sp,PPV,NPV,andOCP,similartothoseofPHOX2B.Resultsareshowninthetablebelow.ThediagnosticperformancetestsofGD2andELAVL4expressiondetectionwerenotablylower.WealsotestedadiagnosticapproachwheresamplesweredefinedaspositiveinthecaseofPHOX2Band/orTHexpressionabovetheTL.IncomparisonwithPHOX2B,theadditionofTHledtohigherDSandNPV,butlowerSpandOCP.Conclusions.PHOX2BandTHhavesimilardiagnosticvaluesforBMinvolvementassessmentinNBpts.ThesimultaneousapplicationofPHOX2BandTHdoesnotbringsignificantbenefitsincomparisonwithPHOX2Balone.GD2andELAVL4expressiondetectionislessrelevantforBMinvolvementassessmentinNBpatients.DSSpPPVNPVOCPPHOX2B0.9601.0001.0000.9770.985TH0.9850.9690.9440.9760.946GD20.4130.9840.9390.7400.772ELAVL40.5730.9610.8960.7920.817PHOX2Band/orTH0.9730.9690.9480.9840.970
