The long-term outcome of BFM-based therapy for childhood lymphoblastic lymphoma in the Moscow region

Lymphoblasticlymphoma(LBL)isoneofpredominantchildhoodnon-Hodgkin’slymphoma(NHL)subtypes.However,excellentoutcomeshavebeenachievedwithacutelymphoblasticleukemia(ALL)-typetreatmentsinchildrenwithLBL.ThepurposeofthisstudywastoinvestigateoutcomeschildrenandadolescentswithLBLtreatedwiththeNHL-BFM90and95protocolsintheMoscowregion.Methods:58pts(m,-40;f,-18)wereenrolledinthisstudyfromMay1991toAugust2008.Fifty-two(90%)ptsweretreatedwiththeALL-liketherapyprotocolNHL-BFM90or95fornon-B-NHL(ALL-type)patients,and6(10%)–withNHL-BFM90forB-NHL.Theseprotocolsdifferedfromtheoriginalbyreductionofthemethotrexatedose(1g/m2/24hinsteadof5g/m2/24h).Results:Themedianageattimeofpresentationwas11.0(range1.5–21.6)years.Forty-five(90%)ptshaveaT-cellimmunophenotype.Fifty-three(91%)hadadvanced(III,IV)stagedisease.ThepresentingsitesofT-LBLincludedmediastinalmassandbonemarrowin35(78%)and13(29%)casesrespectively.CRwas94and83%fornon-B-NHLandB-NHLtreatmentrespectively.Five-yeareventfreesurvival(5y-EFS)was0.80±0.06(medianofobservation4.1years)and0.67±0.19(5.1years)respectively(pgt;0.05).Five-yearoverallsurvival(5y-OS)was0.85±0.05and0.80±0.06respectively(pgt;0.05).ThesituationwithoutmediastinalinvolvementwasafactorforanunfavorableprognosisforT-LBL:5y-EFS–was0.56±0.17vs.0.90±0.05(p=0.036).Sex,age,increasedLDH,sloworfasttherapyresponse,andinvolvementofthecentralnervoussystemorbonemarrowdidnotaffecttheprognosis(pgt;0.05).Conclusions:TheNHL-BFM90and95fornon-B-NHLprotocolsareeffectivetherapeuticregimesforpediatricLBL,andthelong-termresultsobtainedarecomparablewithinternationaldata.
