【 摘 要 】
Background:TheprevalenceofinfectionscausedbyCefotaximase-Munich(CTX-M)-typeextended-spectrumbeta-lactamase-producingEnterobacteriaceae(ESBL-E)hasrapidlyincreasedduringthepast15years.Enterobacteriaceaearecommonlyfoundinthegastrointestinaltractandlong-termintestinalcarriageisconsideredimportantforthespreadofESBLandasasourceofclinicalinfections.OralbiofilmsuchassupragingivalplaqueisknowntocontainnumerousantibioticresistancedeterminantsandmayalsorepresentapoorlyinvestigatedsiteforESBLcarriageandfurtherspread.Objective:ToinvestigatepossiblecarriageofESBL-producingbacteriainsupragingivalplaqueofknownfecalcarriersofthesebacteria.Design:WescreenedforthepresenceofaerobicandanaerobicESBL-producingbacteriaandblaCTX-Minsupragingivalplaquesamplesfromhealthyhumanadultswithculture-verifiedfecalcarriageofCTX-M-producingEscherichiacoli.ThepresenceorabsenceofEnterobacteriaceaeandESBL-producingbacteriainplaquesampleswasevaluatedusingculture-basedmethodsandconsensusCTX-MPCR.Results:Oralsampleswereobtainedfrom17participantswithknownpreviouscarriageofESBL-producingE.coli.NoESBL-producingbacteriaorESBLgenesweredetectedusingculture-basedandmolecularmethods.OnecolonyofRahnellaaquatilisharboringtheclassAESBLgeneblaRAHN-1/2wasidentifiedinanoralsamplefromoneoftheparticipants.Conclusion:ThispilotstudysupportsthenotionthatthepresenceofCTX-M-producingbacteriaisuncommoninoralplaqueofhealthyhumanadultfecalcarriers.Duetothelimitednumberofpersonstested,alowprevalenceoforalESBL-carriageinhealthyadultsorcarriageinselectedgroupsofpatientscannotbeexcluded.Toourknowledge,thisisthefirstdescriptionofanR.aquatiliswiththeRAHN-1/2geneintheoralcavity.Keywords:CTX-M;oralcarriage;extended-spectrum尾-lactamase;Enterobacteriaceae(Published:20August2014)Citation:JournalofOralMicrobiology2014,6:24026-http://dx.doi.org/10.3402/jom.v6.24026ResponsibleEditor:Clay聽B.聽Walker,UniversityofFlorida,USA.*TheChiefEditorIngarOlsen,hasnothadanypartinthereviewanddecisionprocessforthispaper.
