Background:TheantimicrobialpeptideLL-37isasignificantmoleculeofinnateimmunityandrecentstudiesindicatethatitplaysanimportantroleinmaintainingoralhealth.Yetlimitedknowledgeexistsonitsroleinoraldiseasesandorallichenplanus(OLP)inparticular.Objective:Thestudyaimedtoexamine:1)thesalivaryconcentrationofLL-37inpatientswithOLPandhealthysubjects,2)therelationbetweenthetype(reticularorerosive)andsizeofOLPlesionsandLL-37concentration,and3)theeffectofthetherapeuticmodalitiesonLL-37levels.Design:Thesalivarypeptideconcentrationinsamplesfrom20patientsand30healthysubjectsatthesameagerangewasdeterminedbyELISA.Results:Despitethewidevariationinpeptideconcentrationfoundinbothgroups,thehealthysubjectsexhibitedsignificantlylowerlevelsthanpatients.Patientswiththeerosiveformhadsignificantlyhigherpeptideconcentrationsthanpatientswiththereticularform.Systemictreatmentwithcorticosteroidsresultedinasignificantdecreaseofthesalivarypeptideconcentration,whileothertreatmentmodalities,suchasadministrationofvitaminsAandEorlocalapplicationofcorticosteroidshadnoeffect.ImprovedclinicalappearanceofthelesionswasfollowedbyadecreaseinthesalivaryLL-37level.Conclusions:SalivaryconcentrationofLL-37correlatestothemanifestationofmucosalesionsinOLPpatients,thehighestlevelsbeingobservedinthemostseverecases.Thisincreaseinpeptidelevelsmayprotectagainstlesioninfectionandpromoteaquickwoundhealing.Keywords:antimicrobialpeptideLL-37;orallichenplanus;saliva(Published:4December2014)Citation:JournalofOralMicrobiology2014,6:26156-http://dx.doi.org/10.3402/jom.v6.26156
