【 摘 要 】
The patient is a 34 year-old G1P1 Caucasian female withno medical history who was transferred from an outsidehospital after an emergent Cesarean section for increasingjaundice with elevated liver enzymes, abnormalcoagulation times and acute renal failure. The patientwas healthy until 33 weeks gestation when she noteddizziness, headache and visual changes. Blood pressure atthat time was elevated at 140/90 and she had trace lowerextremity edema. No proteinuria was detected. Theclinical picture was consistent with mild pre-eclampsia.The patient was followed until 35 weeks gestation whenshe developed vaginal bleeding and abdominal pain. Atthat time, an emergency C-section was performed dueto a non-reassuring fetal heart rate. On the first nightafter the surgery, she became anuric and laboratory testsdemonstrated acute renal failure, coagulopathy, andelevated liver enzymes. She became progressivelyjaundiced and she was transferred for furthermanagement. On admission, she complained of nauseaand malaise. She denied abdominal pain, chest pain,dyspnea, lightheadedness or headache.
