【 摘 要 】
EarlytreatmentofCLL/SLLdoesnotimpactsurvival‐reflectinglimitationsindetectingprogressionearlyandidentifyingasymptomaticpatientslikelytobenefitfromearlytreatment.ImprovedunderstandingofCLL/SLLbiologywouldidentifybetterprognostic/predictivemarkers.ThisstudyattemptstoaddresstheseissuesbydeterminingtherelationshipbetweencytokineaberrationsandpoorclinicaloutcomesinCLL/SLLinthecontextofagenetic–basedprognosticmodel.Fifty‐nineserumcytokines/chemokinesweremeasuredin28untreatedCLL/SLLpatients.PatientswerestratifiedasGRorint/PRusingcytogenetics.ComparisonofCLL/SLLwith28HCsrevealedincreasedexpressionofTh2cytokines(IL‐10,IL‐5,sIL‐2Rα;P≤0.01)anddecreasedlevelsofTh1cytokines(IL‐17,IL‐23,IFN‐γ;P≤0.003).InamultivariateanalysisofGRversusint/PRgroups,differentialexpressionofsIL‐2Rαmaintainedsignificancewithincreasedexpressioninint/PRCLL/SLL.Withmedianfollow‐upof54.3monthsafterdiagnosis,fourpatientsincurreddiseaseprogression,withanIL‐17/sIL‐2Rαmodelpredictingneedfortreatmentinallcases.Insummary,specificcytokinesignaturesareassociatedwithgeneticallydefinedaggressivediseaseandpredictneedfortherapy.Thissuggestsutilityindetectingdiseaseprogressionearly,identifyingthoselikelytoincurasurvivaladvantagewithearlytreatment,anddirectingfuturetherapy...【 授权许可】
