【 摘 要 】
ClinicaltrialsperformedinpopulationsathighGCriskhavedemonstratedthateradicationofHelicobacterpylorifromthestomachwithacourseofcombinationantimicrobialshelpspreventgastriccarcinogenesiswhentheyareadministeredbeforeprecancerouslesionshaveyetdeveloped.Inthisreview,weconsidertheinsightsintoH.pylori‐associatedgastriccarcinogenesisthathavebeengainedfromtheseandmanyotherclinicalstudiesinthefieldtohighlightpriorityareasforbasicresearchandclinicalinvestigation.AmongthesearedefiningthemagnitudeoftheriskreductionthatmaybeachievedinclinicalpracticeandatapopulationlevelbyH.pylorieradicationandinvestigatingwhen,duringtheslowmultistepprogressiontoGC,interventionwillbeofthemostbenefit.AdditionalstrategiestopreventGCinducedbyH.pylori,includingchemoprevention,dietarymodification,andcloseendoscopicsurveillance,mayalsohavevalueinaugmentingtheriskreduction.WhyonlyasmallsubpopulationofthoseinfectedbyH.pylorigoontodevelopGCmaybepartiallyexplainedbygeneticsusceptibilityrelatedtoSNPsinseveralgenesregulatingtheintensityofthegastricinflammatoryresponsetoH.pylori.InvestigationofthebasicmechanismsunderlyingthepromotionofGCbyH.pyloriandtheassociatedinflammatoryresponsewilllikelycontinuetoimproveclinicalstrategiesforthepreventionofoneofthemostcommoncausesofcancerdeathglobally.Seerelatedreview,AttheBench:H.pylori,dysregulatedhostresponses,DNAdamage,andgastriccancer...
