Effects of alcohol withdrawal on monocyte subset defects in chronic alcohol users | |

Excessivealcoholconsumptionhasamodulatingeffectonimmunefunctionsthatmaycontributetodecreasedimmunityandhostdefense.Itisassociatedwithincreasedintestinalpermeabilitytoendotoxinsthatisnormalizedafter14dofabstinence.WhetherandhowbloodmonocytesubsetsareimpairedinpatientswithanAUDandwhattheirevolutionisafteralcoholwithdrawalarethepaper'sobjectives.Withtheuseofflowcytometry,bloodmonocytesubsetswerequantifiedinAUDsbefore(n=40)and2wkafter(n=33)alcoholwithdrawalandcomparedwithHCdonors(n=20).ExpressionofTLR2andTLR4onmonocytesubsetswasalsoquantified.CytokineresponseofmonocyteswasmonitoredfollowingPGNandLPSstimulation.TheCD14+CD16−subsetwasdecreased,whereastheCD14dimCD16+subsetwasexpanded(P<0.001)inAUDcomparedwithHC.ThefrequenciesofTLR2‐andTLR4‐expressingmonocyteswerereducedinAUDcomparedwithHC.AlthoughthebasalproductionofIL‐1,IL‐6,andTNFbymonocytesinAUDwascomparedwithHC,thePGN‐andLPS‐mediatedIL‐6andTNFproductionwasincreasedinAUD.FrequenciesofIL‐6‐expressingmonocyteswerehigherinAUDthanHC.AlcoholwithdrawalpartiallyrestoredthedistributionofmonocytesubsetsandthefrequencyofIL‐6‐producingmonocytesandincreasedthefrequencyofTNF‐producingcellsinresponsetoLPSandPGNstimulationtolevelscomparedwiththoseinHC.Ourfindingsindicatethatchronicalcoholusealtersthedistributionaswellasthephenotypicandfunctionalcharacteristicsofbloodmonocytesubsets,whicharepartiallyrestoredfollowing2wkofalcoholwithdrawal...
