Implants,transplants,andimplantablebiomedicaldevicesaremainstreamsolutionsforawidevarietyofhumanpathologies.Oneofthepersistentproblemsaroundnondegradablemetallicandpolymericimplantsisfailureofmacrophagestoresolvetheinflammationandtheirtendencytostayinastate,named“frustratedphagocytosis.”Duringtheinitialphase,proinflammatorymacrophagesinduceacutereactionstotraumaandforeignmaterials,whereastolerogenicanti‐inflammatorymacrophagescontrolresolutionofinflammationandinducethesubsequenthealingstage.However,implantedmaterialscaninduceamixedpro/anti‐inflammatoryphenotype,supportingchronicinflammatoryreactionsaccompaniedbymicrobialcontaminationandresultinginimplantfailure.Severalmaterialsbasedonnaturalpolymersforimprovedinteractionwithhosttissueorsurfacesthatreleaseanti‐inflammatorydrugs/bioactiveagentshavebeendevelopedforimplantcoatingtoreduceimplantrejection.However,nodefinitive,long‐termsolutiontoavoidadverseimmuneresponsestotheimplantedmaterialsisavailabletodate.Thepreventionofimplant‐associatedinfectionsorchronicinflammationbymanipulatingthemacrophagephenotypeisapromisingstrategytoimproveimplantacceptance.Theimmunomodulatorypropertiesofcurrentlyavailableimplantcoatingsneedtobeimprovedtodeveloppersonalizedtherapeuticsolutions.Humanprimarymacrophagesexposedtotheimplantablematerialsexvivocanbeusedtopredicttheindividual'sreactionsandallowselectionofanoptimalcoatingcomposition.Ourreviewdescribescurrentunderstandingofthemechanismsofmacrophageinteractionswithimplantablematerialsandoutlinestheprospectsforuseofhumanprimarymacrophagesfordiagnosticandtherapeuticapproachestopersonalizedimplanttherapy...
